  • Monitors and improves the overall quality of services in Nursing provided in the assigned nursing units.
  • Maintains effective communication with interdisciplinary team, residents, families, physicians and staff.
  • Performs activities related to nursing care needs of residents as needed.

As an LPN working in our Skilled Nursing facility you will perform resident care duties and participate in nursing care data collection, planning, implementation and evaluation. You can also expect to:

  • Maintain effective communication with residents, families, staff, and physicians.
  • Complete daily shift documentation, including head to toe review of skilled residents.
  • Function in a calm and positive manner in an emergency or crisis situation.
  • Communicate consistently with the Nurse Manager, reporting pertinent concerns/issues.

Registered Nurse-Skilled Nursing

As a Registered Nurse in Skilled Nursing you will perform resident care duties and participate in nursing care assessments, planning, implementation and evaluation. You will direct all nursing care and provide resident care as assigned.

You can also expect to:

  • Assist with physician visits and recommend changes in treatment based upon resident condition.
  • Administer both medications and treatments according to physician and nursing orders and infection control standards, documenting them at time of delivery.
  • Ensure accurate and timely documentation including, but not limited to, charting, incident reports, admissions and re-admissions, discharges, changes in resident condition, narrative notes, summaries, Individual Service Plan (ISP) updates, MARS, treatment records and nursing care monitoring records.
  • Assist and participate in team conferences in developing goals, priorities and nursing outcome criteria, aimed at meeting the total needs of residents.
  • Conduct resident assessments, admissions, re-admissions and discharges.
  • Ensure communication continuity in resident care on and between shifts.

Child Day Care Teacher

A Child Day Care Teacher:

  • Plans, schedules, implements and supervises the daily program of appropriate developmental experiences for a classroom group of children that meets our center policies as well as state and Youngstar guidelines.
  • Provides care and protection for assigned children and maintains state required ratios. Administers first aid treatment and any medication authorized in writing by parent.
  • Ensures necessary documentation is complete such as daily sheets, meal counts, sign in/out sheets, accident reports, lesson plans, etc.
  • Participates in staff meetings, training sessions, conferences, workshops, and other career development and professional activities. Meets yearly continuing education requirements.
  • Keeps all areas of the center clean and sanitary to maintain infection control and orderly, pleasant surroundings.
  • Provides quality customer service to children and parents, including treating them all with dignity and respect, greeting them and maintaining confidentiality.
  • Participates in Intergenerational Programs as assigned.
  • Meets all physical requirements such as sitting, standing, squatting, bending, twisting, walking, pushing, pulling, moving at a fast pace and lifting sometimes over 50 pounds.

As a CNA in our Skilled Nursing department you will provide direct and indirect care to residents. You will assists in activities of daily living (ADLs) to meet each resident’s needs and provide resident care as assigned.

You can also expect to:

  • Help prevent spread of infection by consistent hand washing, standard precautions and identified isolation procedures in accord with policy and procedure
  • Inform residents of procedure and outcome prior to providing care services
  • Report to staff nurse any unusual behavior or change in resident's condition
  • Monitor and document temperatures, appetite, intake and output as assigned. Weigh residents and record outcomes (Hoyer scale, free-standing, stand or sit scale) on proper forms
  • Use transfer equipment for both resident safety and that of your own: Sit-to-Stand lifts, Mechanical Lifts, and in-bed transfer hardware, gait belts, and bath lifts
  • Watch resident for changes in skin condition, injuries, appetite, emotional needs and other aspects. Report changes to the nurse or supervisor. Take vital signs as directed and record them
  • Use time effectively to accomplish assigned cares for a group of residents
  • Review daily report on assigned residents for their condition and required cares. Use care plan and Resident Information Sheets to maintain current information on resident
  • Perform utility work for resident's comfort and facility neatness and cleanliness:
    • Change bed linens

Cook Assistant

Cook Assistant

As an Assistant Cook you will perform skilled cooking duties that and are responsible for preparing foods for resident needs, and special events.

You can also expect to:

  • Prepare food following food safety guidelines, diet restrictions and  standardized recipes
  • Work off prep sheets and occasionally create prep lists to ensure all menu items are prepared
  • Plan cooking schedules so that foods are ready at specified times and are tasty and presentable
  • Procures food products from refrigerators, freezers, or other storage areas
  • Assist Cook with meal service and serve food according to portion control guides and diet restrictions
  • Measure and mix ingredients according to recipes and able to use a variety of kitchen utensils and equipment, such as blenders, mixers, grinder, and slicer to make sauces, soups, stews, casseroles, and desserts
  • Bake, roast, broil, grill and steams meats, fish, vegetables, and other foods properly for good taste and appearance
  • Observe and taste food being cooked and adds ingredients or seasonings to improve flavor
  • Review food in coolers and refrigerators for outdated items
  • Basic knowledge of diets
  • Wash pots and pans as needed
  • Takes late requests and prepare late tray orders
  • Operate electric equipment and kitchen utensils in a safe and efficient manner using appropriate P.P.E.
  • Operate equipment, utensils, and counters in accordance with sanitary standards and cleaning schedule
  • Check and make appropriate arrangements for availability of equipment and food for the next day’s menu
  • Complete all other duties as assigned by supervisors

Dining Attendant

Dining Attendant

The Dining Attendant is a Part-Time Position.  As a Dining Attendant in our Skilled Nursing or Assisted Living departments you will assist in preparing and delivering residents trays and table setting by placing silverware, napkins, placemats on table and on trays.

You can also expect to:

  • Restock and keep coffee cart clean, set up coffee cart and make coffee
  • Fill condiment containers for each cart, and keep condiment filled in dining room
  • Restock milk cooler in Dining Room after each meal and keeps cooler clean
  • Take tray carts to dining rooms, cup racks and glass racks
  • Assist in serving residents trays in dining room when needed
  • Operate dishwasher to ensure thorough cleaning of silverware and utensils using proper washing procedure
  • Empty and store clean dishware and utensils in proper places     
  • Remove soiled dishes from dining room and brings to dishwashing area
  • Monitor and record dishwasher temperatures
  • Keep all equipment, working areas and floors clean and orderly as per cleaning schedule
  • Wipe off tray cards and place in proper order for next meal service
  • Report any needed supplies, faulty looking equipment, accidents, problems, etc…
  • Sweep floor daily, wash mats and wash floors when needed
  • De-lime dish machine (1) day a week
  • Fill in for Cold Prep Dietary Aide as needed
  • Any other duties as assigned by management
