Hospital and Community Liaison

The Hospital and Community Liaison serves as a dependable point of contact for all utilization of services and communicates programming and service needs. Additionally, this position requires advocating and promoting the facility’s services to hospitals, case workers, discharge planners, physician offices, and other clinical providers. The Hospital and Community Liaison follows up on all hospital referrals, evaluates proper placement, explains the facility’s services, and maintains and develops referral sources.

  • Plans and executes the community marketing and communications on and off campus.
  • Facilitates and assures the delivery of discharge reports and communication.
  • Coordinates all public relations functions.
  • Assists in the facility’s advertising efforts.
  • Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.

 


Job Requirements

 

  • Sales Experience required
  • LPN or social worker preferred
  • A minimum of two (2) years experience in nursing, home health preferred
  • Excellent oral/written communication skills
  • Demonstrated ability to provide a high level of customer service
  • Ability to handle a fast paced environment with a high level of organization
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills
  • Computer proficiency required
