Activities Assistant

The Gardens at St. Elizabeth is searching for an Activities Assistant for our fabulous seniors! 

Scenic Drives, Educational Speakers, Wii Bowling and BINGO barely scratch the surface! If you are passionate about exceeding expectations in the provision of quality activities programs we are looking for you!                

The Activities Assistant provides individual and group activities appropriate to the needs and interests of each resident to promote well-being and quality of life. The Activities Assistant may care for the following age specific population: adults and older adults.

  • Assists in planning, scheduling, and implementing an on-going program of activities designed to meeting, in accordance with the comprehensive assessment, the interests and physical, mental, and psychosocial well-being of each resident, including both individual and group activities.
  • Completes the Resident Activity Assessment in accordance with facility, federal, and state guidelines and documents the activity interests and needs of each resident.
  • Assists and implements activities for days, evenings, weekends, and holiday. Assists in organizing and scheduling community events related to resident needs and interest.
  • Visits room-bound residents one-on-one on a daily basis, or as needed.
  • Maintains up-to-date knowledge and complying with policies and procedures of infection control, OSHA, safety, and federal and state laws and regulations. Interacts effectively within multi-disciplinary teams.
  • Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.CHI Living Communities has the right to change the job specific duties and specifications required for the position of Activities Assistant from time to time without prior notice.
  • In addition to the essential job specific duties listed above, the Activities Assistant shall be required to perform all duties (essential and non-essential) in a manner consistent with the mission statement and core values (reverence, integrity, compassion, excellence) of CHI Living Communities and will be evaluated on such basis. LPN

The Licensed Practical Nurse (LPN) provides resident care in accordance with the Nurse Practice Act and is responsible for the delivery of safe and therapeutic resident care from admission through discharge in collaboration with the physician, family, and other members of the multi-disciplinary health care team. The LPN is responsible for utilizing principles of the nursing process in carrying out delegated aspects of care for both residents and families. The goal of resident care shall be toward restoring and maintaining each resident’s identified capabilities at their maximum mental and physical level.

Receptionist

Essential Job Specific Duties/Responsibilities:

  1. Provides administrative support to facility staff and residents.
  2. Greets all who enter facility.
  3. Assists with incoming calls.
  4. Distributes mail.
  5. Processes the Nurse Aide Registry.
  6. Records in-service attendance.
  7. Schedules appointments and transportation for residents.
  8. Assists other departments with various paperwork, brought to his/her workstation, as time allows (i.e. medical records, social services, supplies, etc.).
  9. Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.
  10. Attends all mandatory in-service programs.
  11. Adheres to facility policies and procedures.
  12. Completes yearly online education program by deadline.
  13. Attends employee meetings on a regular basis.

In addition to the essential job specific duties listed above, the Receptionist shall be required to perform all duties (essential and non-essential) in a manner consistent with the mission statement and core values (reverence, integrity, compassion, excellence) of CHI Living Communities and will be evaluated on such basis. Resident Assistant - PRN

POSITION ESSENTIAL FUNCTIONS:

  1.  Provides resident care and services as assigned and, as provided for in the care plan, to assure the safety and comfort of residents, including but not limited to, assistance with bathing, shaving, dressing, laundry, escorting residents to meals and social activities, tidying their apartments, assisting them with their activities of daily living, taking vital signs, doing peri-care, skin care, oral care, reminders of or assistance with toileting, appointments, and help with using their oxygen.
  2. Works well as a team member, displaying flexibility with assignments and a willingness to assist co-workers. 
  3. Follows policies and procedures without prompting.
  4. Follows all safety guidelines, including OSHA health and safety rules and regulations, and other safety procedures.  Attends and participates in training and education sessions as scheduled, including staff meetings, in-services, and any other staff development opportunities.  Always uses the safety equipment and procedures provided when performing hazardous tasks.
  5. Maintains a professional manner at all times when interacting with residents or their families or visitors.
  6. Communicates in an effective and timely manner to appropriate people regarding resident needs and services, departmental issues and family concerns, through consistent verbal or written follow-up.
  7. Assists residents with observance of physical and medical precautions following a hospital stay, therapy treatment or a life-threatening event.
  8. Must be able to assist residents physically throughout the shift.  Reports to and assists the LPN or charge person on the shift, as needed.
  9. Promotes resident and staff safety through the use of proper body mechanics, and through frequent rounds on assigned areas.  Reports resident needs, status changes and safety issues promptly.
  10. Additional duties may be outlined by facility.  Duties and responsibilities may be added, deleted, or changed at any time at the discretion of management formally or informally, either verbally or in writing.
