Associate Director of Facilities

Position Summary

The Associate Director of Facilities will be responsible for the following areas:

  • Planning, organizing, and directing St. Leonard’s facilities program, including the maintenance, security, grounds, and transportation departments
  • Ensuring compliance with federal, state, local, and regulatory/accrediting agencies, including the Ohio Department of Health and the Joint Commission
  • Serving as the campus Safety Director, including chairing the Safety Team Committee and documenting meetings and actions items in accordance with Joint Commission requirements
  • Conducting regular safety, equipment, grounds, and facilities rounds and inspections and documenting results and any needed action items
  • Ensuring disaster plans are documented, contracts with emergency supply vendors are in place, and regular disaster drills are conducted in accordance with regulations
  • Ensures work orders are addressed within a 24 hour period, or sooner as required, and ensures follow up action and communication in the event of a delay

Dining Attendant

As a Dining Attendant you will assist in preparing and delivering residents trays and table setting by placing silverware, napkins, placemats on table and on trays.

You can also expect to:

  • Restock and keep coffee cart clean, set up coffee cart and make coffee
  • Fill condiment containers for each cart, and keep condiment filled in dining room
  • Restock milk cooler in Dining Room after each meal and keeps cooler clean
  • Take tray carts to dining rooms, cup racks and glass racks
  • Assist in serving residents trays in dining room when needed
  • Operate dishwasher to ensure thorough cleaning of silverware and utensils using proper washing procedure
  • Empty and store clean dishware and utensils in proper places     
  • Remove soiled dishes from dining room and brings to dishwashing area
  • Monitor and record dishwasher temperatures
  • Keep all equipment, working areas and floors clean and orderly as per cleaning schedule
  • Wipe off tray cards and place in proper order for next meal service
  • Report any needed supplies, faulty looking equipment, accidents, problems, etc…
  • Sweep floor daily, wash mats and wash floors when needed
  • De-lime dish machine (1) day a week
  • Fill in for Cold Prep Dietary Aide as needed

The Director of Nursing will be responsible for the following areas:

Monitors and improves the overall quality of services in Nursing provided at the Health and Rehabilitation Center (HRC).

  1. Maintains effective communication with interdisciplinary team, residents, families, physicians and staff.
  2. Performs activities related to nursing care needs of residents as needed.
  3. Participates in the development, implementation and evaluation of facility performance improvement program.
  4. In collaboration with the Administrator, operates the HRC in accordance with established policies and procedures of St. Leonard.
  5. In collaboration with the Staff Development Nurse, develop, plan, conduct and schedule in-service training classes that ensure a well-educated nursing service department.

Groundskeeper

 Groundskeepers are required to perform all tasks required to maintain the grounds, gardens, walks and roads in a safe, appealing condition.  All personnel are on-call for snow and ice removal in the winter.

Essential Job Specific Duties/Responsibilities

  • Groundskeepers will operate all equipment necessary to mow, trim, prune, plant and spray grass, trees and shrubbery.
  • Groundskeepers will maintain all roads, walks and outdoor lighting in a safe working condition, free of hazards.
  • Groundskeepers will perform routine maintenance and repairs on all equipment and vehicles, along with appropriate documentation.
  • Groundskeepers will perform a variety of tasks assigned by work orders that relate to the grounds and landscape.
  • Groundskeepers will take part in a variety of inside maintenance tasks and projects.

Licensed Practical Nurse - LPN

Under supervision of the Registered Nurse, the Licensed Practical Nurse (LPN) provides resident care in accordance with the Ohio Nurse Practice Act.  The LPN is responsible for utilizing principles of the nursing process in carrying out delegated aspects of care for both residents and families.  The goal of resident care shall be toward restoring and maintaining each resident’s identified capabilities at their maximum mental and physical level. Essential Job Specific Duties/Responsibilities

 

  1. Maintains effective communication with residents, families, staff and physicians.
    1. Responsible for daily shift documentation including head to toe review of skilled residents.
    2. Functions in a calm and positive manner in an emergency or crisis situation.
    3. Communicated consistently with the RN Unit Manager; reports pertinent concerns/issues to RN Unit Manager.
    4. Places necessary calls for families and physicians. Demonstrates appropriateness of faxing vs. phone calls to physicians.
    5. Communicates appropriately with physicians; makes rounds with physicians in absence of RN Unit Manager or as delegated by RN Unit Manager. Utilizes physician file appropriately.
    6. Reviews and teaches resident and family about pain management, including pain-rating scales and goal setting; importance of aggressive, preventive pain treatment; analgesic misconceptions; and plan for pain management.
    7. Assists RN in developing, implementing and evaluating teaching plan to meet learning needs of residents and families.
  1. Documents in accordance with nursing policies and procedures.
    1. Documentation reflects interventions and resident response; resident/family teaching documented in Nursing Notes and Resident Plan of Care.
    2. Accurately records review of physiological and mental dimensions of residents, including pain review.
    3. Monthly summary completed accurately and timely utilizing resident plan of care.  Plan of Care and Resident Care Card updated/revised monthly and as necessary.
    4. Skin integrity reviews accurately reflect resident and are completed weekly.
    5. Documents resident response to non-pharmacological strategies to promote pain relief and resident response to analgesics.
    6. Documents, at time of service, all medications and treatments administered.
    7. Documents in EMR accurately and timely.
  1. Responds to resident needs with appropriate interventions with the licensure of an LPN.
    1. Provides nursing interventions according to the written plan of care.
    2. Gives and receives a thorough report to/from oncoming nurse.
    3. Monitors work performance of nurse assistants. Make frequent rounds throughout shift.
    4. Suggests appropriate ways to solve problems and make improvements on the unit(s).
    5. Provides hands-on direct care; including ADL’s and treatments as necessary. Able to delegate specific nursing tasks appropriately to STNA’s, excluding anything that requires nursing judgments.
  1. Performs activities related to medications and treatments in accordance with facility policies and procedures.
    1. Safely administers medications and treatments ordered by the physician.
    2. Maintains control of scheduled drugs and all drugs in medicine cart; monitors for discrepancies and reports promptly to RN Unit Manager.
    3. Keeps medication cart key-locked when not within eye view of nurse.
    4. Appropriately records and orders medications from the pharmacy.
    5. Monitors for and reports Adverse Drug Reactions appropriately.

Resident Assistant - Full Time

As a Resident Assistant in our Assisted Living community you will provide direct and indirect care to residents under nurse supervision. Assists in activities of daily living (ADLs) to meet each resident’s needs and you can also expect to assist with:

  • Feeding and assistance with eating
  • Bathing and weighing
  • Dressing
  • Oral hygiene
  • Nail, hair, skin care and shaving
  • Toileting
  • Restorative Nursing: ROM, ambulation, positioning, splint application
  • Prevent spread of infection by consistent hand washing, standard precautions and identified isolation procedures in accord with policy and procedure
  • Inform residents of procedure and outcome prior to providing care services
  • Report to staff nurse any unusual behavior or change in resident's condition
  • Monitor and document temperatures, appetite, intake and output as assigned. Weigh residents and record outcomes (Hoyer scale, free-standing, stand or sit scale) on proper forms
  • Use transfer equipment for both resident safety and that of your own: Sit-to-Stand lifts, Mechanical Lifts, and in-bed transfer hardware, gait belts, and bath lifts
  • Consistently use best practice body mechanics during lifting and handling for your safety and that of residents and ask for help in handling when you are in need
  • Watch resident for changes in skin condition, injuries, appetite, emotional needs and other aspects. Report changes to the nurse or supervisor. Take vital signs as directed and record them.
  • Use time effectively to accomplish assigned cares for a group of residents
  • Review daily report on assigned residents for their condition and required cares. Use care plan and Resident Information Sheets to maintain current information on resident.
  • Perform utility work for residents' comfort and facility neatness and cleanliness:
    • Change bed linens
    • Keep closets, dressers, bedside cabinets
    • Clean and properly maintain sanitation and tub rooms

Resident Assistant - Full Time Day Away

The Security Officer is responsible for the security and safety of all residents, staff and employees. Security Officers monitor all emergency and life safety systems. Security also provides help and contacts for after-hours for Maintenance and other departments.
