Job Summary

The Certified Nursing Assistant assists residents in maintaining and possibly increasing their level of independence whenever possible.

Essential Job Specific Duties/Responsibilities

  1. Provides/assists residents with hygiene needs.
  2. Provides/assists residents with bathing, showering, and dressing needs.
  3. Assists residents with hair care, nail care, oral hygiene (denture and mouth care), and shaving needs.
  4. Assists residents in meeting their hydration and nutritional needs.
  5. Prompts/assists residents, including wheelchair dependent, to the dining room for meals as appropriate.
  6. Serves meal trays to residents in their room promptly and efficiently.
  7. Assists/feeds residents when necessary.
  8. Responsible to see that nourishment/fluids are passed at the appropriate times.
  9. Measures I&O and meal information and records in EMR.
  10. Provides ice water to residents and refills pitchers as needed.
  11. Assists residents to attain or maintain their highest physical, mental, and psychosocial function.
  12. Reports to Charge Nurse upon arrival to determine plan of care for shift and gives report to Charge Nurse at end of shift.
  13. Assists with ambulation and/or transportation needs of resident.
  14. Encourages participation in facility’s activities program.
  15. Reports changes in physical or mental functioning to Charge Nurse.

Certified Nursing Assistant-Skilled Nursing

As a CNA in our Skilled Nursing department you will provide direct and indirect care to residents. You will assists in activities of daily living (ADLs) to meet each resident’s needs and provide resident care as assigned.

You can also expect to:

  • Help prevent spread of infection by consistent hand washing, standard precautions and identified isolation procedures in accord with policy and procedure
  • Inform residents of procedure and outcome prior to providing care services
  • Report to staff nurse any unusual behavior or change in resident's condition
  • Monitor and document temperatures, appetite, intake and output as assigned. Weigh residents and record outcomes (Hoyer scale, free-standing, stand or sit scale) on proper forms
  • Use transfer equipment for both resident safety and that of your own: Sit-to-Stand lifts, Mechanical Lifts, and in-bed transfer hardware, gait belts, and bath lifts
  • Watch resident for changes in skin condition, injuries, appetite, emotional needs and other aspects. Report changes to the nurse or supervisor. Take vital signs as directed and record them
  • Use time effectively to accomplish assigned cares for a group of residents
  • Review daily report on assigned residents for their condition and required cares. Use care plan and Resident Information Sheets to maintain current information on resident
  • Perform utility work for resident's comfort and facility neatness and cleanliness:
    • Change bed linens
    • Keep closets, dressers, bedside cabinets
    • Clean and properly maintain sanitation and tub rooms
    • Separate soiled clothing, linens, and disposables
  • Meet resident's personal needs at least every two hours in the following areas, including, but not limited to:
    • Toileting
    • Repositioning
    • Safety Intervention Devices
  • Provide direct resident care and assistance in ADLs by:
    • Feeding and assistance with eating

  1. Responds to resident needs with appropriate interventions with the licensure of an LPN.
    1. Provides nursing interventions according to the written plan of care.
    2. Gives and receives a thorough report to/from oncoming nurse.
    3. Monitors work performance of nurse assistants. Make frequent rounds throughout shift.
    4. Suggests appropriate ways to solve problems and make improvements on the unit(s).
    5. Provides hands-on direct care; including ADL’s and treatments as necessary. Able to delegate specific nursing tasks appropriately to STNA’s, excluding anything that requires nursing judgments.
  1. Performs activities related to medications and treatments in accordance with facility policies and procedures.
    1. Safely administers medications and treatments ordered by the physician.
    2. Maintains control of scheduled drugs and all drugs in medicine cart; monitors for discrepancies and reports promptly to RN Unit Manager.
    3. Keeps medication cart key-locked when not within eye view of nurse.
    4. Appropriately records and orders medications from the pharmacy.
    5. Monitors for and reports Adverse Drug Reactions appropriately.

Providence Care Center, a continuing care retirement center in Sandusky, Ohio, is seeking a full-time Spiritual Care Coordinator.   3. Schedules and coordinates liturgical services and chapel usage, including weekly Mass and Protestant services. Plans and conducts Liturgy of the Word with Communion Services for Catholic residents and facilitates Prayer Services for Protestant residents when a priest or minister is unavailable.
  4. Completes a spiritual assessment on each resident within seven days of admission and documents all visits in the resident’s medical chart and in the proper file in the Spiritual Care Office.
  5. Prioritizes work in order to be available for regular one-on-one visits with residents and responds to referrals to residents who may be in need of special care, i.e., end of life care, bereavement or change in condition.
  6. Visits residents who are admitted to the hospital within three days.
  7. Coordinates and schedules spiritual care volunteers and Extraordinary Ministers of Holy Communion.
  8. Plans and facilitates twice yearly memorial services for residents who have recently died.
  9. Serves on the Ethics Committee and attends the meetings of the area Ministerial Association. Serves on facility teams and committees as appropriate.
  10. Collaborates with the Director of Mission Integration on mission initiatives.
  11. Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.
  12. Attends all mandatory in-service programs.
  13. Adheres to facility policies and procedures.
  14. Completes yearly online education program by deadline.
  15. Attends employee meetings on a regular basis.
